Wyoming - Apply for Duplicate Driver License or Identification Card

Procedure

1. Visit your local driver exam office in person.
2. Fill out the Application Form.
3. Present a valid U.S. passport or state-certified birth certificate.
4. Provide proof of a name change (such as a marriage certificate, divorce decree, court order, etc.) if your name is now different or has changed from your birth certificate or U.S. passport.
5. Present proof of your Social Security Number (W-2 form, IRS tax form, SSN card, etc.)
6. Present two (2) current forms of proof of Wyoming residency ( See below for more information on proof of residency.)
7. Pay the required fees.

If you've lost your Wyoming driver license or identification card, and you are not currently in Wyoming, you must

1. Print and complete the Application Form. (If you have questions, you may contact Customer Service at 307-777-4800 for assistance).
2. Send the form to: Driver Services - WYDOT, 5300 Bishop Blvd., Cheyenne, WY 82009-3340, with the appropriate renewal fee: $15 class "A, B or C" nonCDL, $20 for CDL, add $3 if you have a motorcycle class code.
3. If you are a minor, under the age of 18 years, your legal parent or guardian must sign the minor's release in front of a notary and the notary must certify their signature on the back of the application form.
4. Include two (2) current forms of proof of Wyoming residency.
5. Please allow 60 days to process your application.

Required Documents

Proof of Identity

• Valid U.S. passport or state-certified birth certificate
• Proof of Social Security Number (W-2 form, IRS tax form, SSN card, etc.)
• Two (2) current forms of proof of Wyoming residency
• Proof of a name change (such as a marriage certificate, divorce decree, court order, etc.) if your name is now different or has changed from your birth certificate or U.S. passport.
• Documentation from your physician certifying completion of all operations necessary to effect a change of sex

Proof of Social Security Number - You are required to present (1) of the following as proof of your Social Security number

• Social Security card,
• Bank Statement with your complete SSN on it,
• W-2 form with your complete SSN on it, OR

Any other government or official document that displays your complete Social Security number. Proof of Residency

You are required to present any two (2) of the below acceptable documents to prove you are a Wyoming resident:

• Current utility bill with your name and residential (not P.O. box) address on it;
• Current rental or lease agreement with your name and residential (not P.O. box) address on it;
• Certified voter registration certificate (containing your name and residential address - not P.O. box) displaying the stamp of the current county clerk for that county; or
• Vehicle registration in your name with your Wyoming residential address (not P.O. box) on it.

If you do not have these documents, you must contact the Cheyenne headquarters help desk at 307-777-4810 for assistance and information concerning other possible acceptable documents that could prove you are a resident of Wyoming.

Renewal Driver License & Permit Fees:

• $15 - Class A, B, or C nonCDL
• $20 - Any class CDL (Commercial Driver License)
• $20 - Any class CLP (Commercial Learner's Permit)
• $3 - Temporary Motorcycle Permit

Validity

Identification cards issued after Jan. 1, 2010 are valid for a maximum of eight years.
