- The applicant has to fill-in the details in the form.
- Applicant should make sure to send all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
- Mail the application to appropriate office.
- Mailing address is mentioned in “Office Locations & Contacts” of this page.
- There is no charge for this procedure.
- Concerned authorities will look into the application form and submitted documents.
- The authorities will check whether or not the organisation or association has been certified will be communicated by the U.S. Department of Agriculture (USDA).
- No need to reapply for organisations that are already certified to nominate board members.
- The Board will just have 30 days to decide whether or not to grant the exemption request.

EligibilityEdit
- To market or handle lamb or lamb products in the United States, the applicant must be a producer, feeder, seed stock producer, or first handler who pays the necessary assessment.
- A certified producer group must recommend the applicant and they must submit a full application.
FeesEdit
There is no fee required for this procedure.
ValidityEdit
- All Board members are elected for three-year terms.
- No member of the Board will serve more than two three-year terms in a row.

- This form determines if organisations are eligible to nominate lamb farmers, seed stock producers, feeders, and first handlers to serve on the Lamb Promotion, Research, and Information Board (Board).

The information needed to be filled in the application form are listed below:
- Name and address of organization
- Telephone
- Type of organization
- Total active membership
- Number of active members engaged lamb production feeding or slaughter
- Total estimated "producer" sales of first handler purchase of lamb by active members
- As evidence of the stability and permanency of the organization
- List the source(s) from which the organization's operating funds are derived
- Describe the functions of the organization
- Describe the organization's ability and willingness to further the purpose and objectives of the lamb order
- Describe the geographic territory covered by the active membership of the organization
- Describe the nature or make-up of your active membership. Indicate the proportion of the total number of producers, seed stock
- Producer, feeders, and first handlers in the state or geographic area served which are active members of the organization
- Signature
- Date
- Name and title of person completing this application
