Procedure

Apply by Post

1. Complete an application form (MDL26 form). Replacement/certified copy of Drivers License, application (Form DL26): http://www.transport.wa.gov.au/mediaFiles/LBU_F_DL_DL26_DuplicateLicence.pdf
2. Post completed form and supporting documents. Applications should be posted to:
   Department of Transport
   Driver and Vehicle Services
   GPO Box R1290
   Perth WA 6844
3. Your application will be processed. Your replacement driver's license will be processed and post to the address currently recorded.

Apply in Person

1. You can apply in person by attending a Driver and Vehicle Services Centre or regional Agent.
2. To speed up the process it is recommended that you bring the completed form MDL26 with you. However if you are unable to do this, you can complete the form at a Driver and Vehicle Services Centre or regional Agent.

Required Documents

• Completed application form (MDL26)
• Certified copies of primary and secondary proof of identification. One of these must show your signature. (Examples of accepted documents are provided in the Proof of identity requirements fact sheet)
• Police incident report number - for stolen driver's license documents only
• Payment - for lost, damaged and replacement driver's license documents only.

Eligibility

Anyone in Western Australia who holds a driver's license.

Fees

• Duplicate/replacement drivers license - $33.05
• Duplicate/replacement drivers license (Pensioners and Seniors) - $16.50
• Duplicate/replacement drivers license (Aged Pensioners) - No Fee

Instructions

• To replace your driver's license document you will need to complete an application form (MDL26 form below). The form can be submitted in person or by post.
• You can receive a replacement driver's license document free of charge by supplying a Police incident report number along with a completed application.
• Applying by post applies to driver's licenses where we have an existing photograph and signature on record.
