Procedure

1. Complete the change of personal details form. You can download the form here: http://www.transport.wa.gov.au/mediaFiles/LBU_F_P64_PersonalDetails.pdf
2. Provide the required documents stated below. (See Required Documents section)
3. Post the completed form. Post the completed form with supporting documentation to:
   Department of Transport
   Driver and Vehicle Services
   GPO Box R1290
   Perth WA 6844.

Required Documents

• Certified copies of one primary and one secondary proof of identification documents (refer to fact sheet below on acceptable forms of identification)
• A Certificate of Recognition issued by the Gender Reassignment Board. The certificate is not valid until one month after the issue date and cannot be accepted until that period of time has lapsed.
• If you request a change of gender due to reassignment but not in possession of a Certificate of Recognition, please contact the Gender Reassignment Board

Eligibility

Anyone in Western Australia who holds a driver's license.

Fees

There is no fee payable to change your name, address or gender on our system. However a fee will apply if you request a replacement drivers license document showing your new name or address (rather than using the change of address sticker), or wish to change the photograph recorded on your license document.

Validity

Explain the time until which the certificate/document is valid.
e.g. Birth Certificate Valid Forever

• Full Name
• Gender
• Date of Birth
• Residential Address
• Postal Address
• Contact Number
• License number(if applicable)
• firearm license number (if applicable)
• boat registration number (if applicable)

Need for the Document

If you have undergone a gender reassignment, you must provide approved proof of identity documents to support the change to the Driver and Vehicle Services database.
