Procedure

1. Complete the Change of personal details (Form P64). You can download the form here: http://www.transport.wa.gov.au/mediaFiles/LBU_F_P64_PersonalDetails.pdf.
2. Provide the necessary and required with your application as stated below.
3. Post the completed form. Post the completed form with supporting documentation to:
   Department of Transport
   Driver and Vehicle Services
   GPO Box R1290
   Perth WA 6844.

Required Documents

• Certified copies of any documentation relating to your name change: A marriage certificate (Commemorative Certificates are not accepted), a divorce decree (bearing the name being reverted to), a deed poll registered with the relevant authority, a change of name certificate issued by the Registry of Births, Deaths and Marriages, a birth certificate showing the name at birth and the new name (Commemorative Certificates are not acceptable).
• In addition to an acceptable change of name document you must provide certified copies of the drivers license document to be changed. The original drivers license document must be produced if changing your name in person.
• Your identity and change of name documents should show a clear link between your names.
• At least one of the documents should contain your signature.
• Change of name documents are not accepted via fax.

Eligibility

Anyone in Western Australia who holds a driver's license.

Fees

There is no fee payable to change your name, address or gender on our system. However a fee will apply if you request a replacement drivers license document showing your new name or address (rather than using the change of address sticker), or wish to change the photograph recorded on your license document.

Validity

Explain the time until which the certificate/document is valid.

Instructions

• The information you supply may be disclosed to other government agencies where provided for in legislation. The Director General of the department also releases aggregated statistical information to third parties. However, your personal identifying information will not be released to these persons without your explicit consent.
• If you have a postal address, which is different to your residential address, it can be used for correspondence purposes but you must provide a residential address. To obtain a new driver's license complete with your updated name change you will need to attend a Driver and Vehicle Services Centre so that your new signature can be recorded and added to your new license.
