ProcedureEdit

  1. Determine if you are eligible. You must have an F extension (previously called F endorsement) on your driver's license to drive a vehicle for Hire and Reward.
  2. Attend a Driver and Vehicle Services Centre or regional Agent. To apply for the F extension you will need to provide the following original documents at the time of application.
  3. Undergo and pass a medical examination by your doctor. You will need to book and attend a medical examination by your doctor. You will need to take the Medical assessment certificate - fitness to drive form with you.
  4. The Department of Transport will assess your application and you will be advised in writing of the outcome. If approved advice on the procedure will be provided.

  • National Police Certificate (application can be made at Australia Post or via the WA Police website); and
  • Traffic Conviction record and Traffic Infringement record Applications for traffic records can be made at a police station or via the WA Police website
  • Completes License application form,
  • names and contact details of two referees who are not family members and who have known you for at least four (4) years
  • primary and secondary identification documents
  • Completed eyesight test and application form
  • Payment

  • have held a relevant driver's license for at least four (4) years;
  • be of good character; and
  • be mentally and physically fit to drive a motor vehicle for the purposes of carrying passengers for reward.

InstructionsEdit

  • The medical assessment certificate must be completed and returned to us within three (3) months of the application date for your F extension. Medical assessment certificates received after that date will be refused. You will be advised of this in writing and if you still wish to hold an F extension you will need to make a new application by completing all of the steps.
  • Your 'F' extension endorsement will automatically cease to be valid if you do not complete and pass the periodic medical assessment as prescribed in the Road Traffic (Authorisation to Drive) Regulations 2008.
