Apply In-Person:

  1. The candidates shall approach their respective Government Medical College / District Hospital / Government Hospital to ensure their eligibility by getting themselves examined.
  2. Contact link in the given page, please select the “MEDICAL INSTITUTIONS” option on the left part of the screen and choose the respective sub option as appropriate and click on the desired option for contact details.
  3. Please visit the respective institution. If the institution is capable or designated to examine a candidate, please get examined as per the norms.
  4. The institute will examine the candidate to assess the disability as per the diagnostic norms.
  5. Based on the outcome of the examination, the respective institution will issue the report with details about the disability within 5 days or as per the timeline the tests and lab related examination takes.
  6. With the report in hand, the candidate shall visit the respective disability assessment board as appropriate to get the physically handicapped certificate.
  7. The candidates are advised to adhere to the announcement of the respective department that offers reservation of seats for admission in educational sector or in recruitment for the authorized disability assessment board for getting assessed to be eligible for reserved seats.
  8. The candidates shall refer the respective departments (that offered reservation of seats for admission in educational sector or for recruitment) announcement for the authorized disability assessment board for getting assessed to be eligible for reserved seats.

  10. Please visit the respective disability assessment board that issue certificate as appropriate.
  11. The candidate shall obtain the application form from the respective office or write on a plain A4 sheet as advised.
  12. The candidate shall use the following link to download the application form link please refer to the page number 5. Please use the application form after getting it authenticated from the department.
  13. Please complete the application form and attach all the documents as per the “Required Documents” section of this page and submit it to the designated authority.
  14. Authorities will check the submitted application and documents to make sure that the required documents and details are in place. If the documents and details are in place, the application will be accepted.
  15. Please pay the required Fees as per the authority’s request.
  16. ”SMS” triggered update on application status will be sent to the applicant’s mobile.
  17. This application will be processed internally. The submitted documents and reports will be scrutinized. The certificate will be sanctioned based on satisfactory grounds.
  18. The status, viz. Approval will be conveyed to the applicant through an “SMS” or by post or as per applicable norms.
  19. The applicant shall visit the respective office as notified and follow the authorities guidelines to get the certificate.
  20. The certificate will be delivered to the office address mentioned on the application through post or hand-delivered as notified, or it will be delivered as per applicable norms.
  21. The whole process can be completed within one week or as per the timeline provided by the authorities.

Need for the DocumentEdit

  • A physically handicapped (PH) certificate is required for the deserving candidate to avail the government benefits, especially for admission in educational courses or to get recruitment based on reservation of seats for physically handicapped quota.
  • The successful NEET examination candidates with the expected percentage of disability shall obtain this certificate in order to get the seat reserved for such candidates.
  • The Constitution of India promotes equality, freedom, justice and dignity of all individuals and implicitly mandates an inclusive society for all including persons with disabilities.
  • According to the provisions of the Persons with Disabilities Act of 1995, it has identified seven categories of disabilities; blindness, low vision, leprosy cured, hearing impairment, loco-motor disability, mental retardation and mental illness.
