Virginia - Obtain a Replacement Driver License

Procedure

In Person

1. Go to any of Virginias Customer Service Center. You will be required to pass the vision screening without wearing glasses or contact lenses.
2. Complete the Driver's License and Identification Card Application.
3. Present the necessary documents.
4. Submit a new photo.
5. Pay the fees.

By Mail

1. Complete the Driver's License and Identification Card Application.
2. Include a Vision Screening Report (MED 4) that has been signed by an ophthalmologist or optometrist within the last 3 months.
3. Mail both the completed application form and vision screening report to:
   Department of Motor Vehicles
   Driver Support
   P. O. Box 27412
   >Richmond, VA 23269-0001
4. Driver Support will process your request and send you a replacement license by mail with the vision restriction removed.

Apply Online

1. Go to VA DMV Online Services.
2. Use A DMV issued PIN number.
3. Fill in the required information.
4. Pay through a valid credit card.
5. Your old photo will be used.

Instructions

• If your driver license was lost, damaged or stolen, you'll need to replace it. You can also replace it when you need to change your address, change organ donor status, update the photograph on your license or learner's permit, or replace your Social Security Number with a DMV-assigned customer number.
• If the expiration date on your license is less than six months away, you may choose to renew your driver's license instead of replacing it.
