Send the completed, notarized application TO THE DISTRICT WHERE THE BIRTH OCCURRED.

Do not forget to include the following:
- Photocopy of your valid, picture identification document.
- Certified copy of court/appointment document if applying as legal representative or legal guardian.
- United States Postal Money Order in the amount of $15.00, made payable to the Virgin Islands Department of Health. (NO PERSONAL CHECKS WILL BE ACCEPTED)
- Stamped envelope in the amount of $5.60 for return by certified mail or $18.30 for return by express mail.

EligibilityEdit
You may obtain this type of certificate if:
- You are the person listed on the record and you are at least 18 years.
- You are the parent or legal guardian of the minor person listed on the record.
- You are a party entitled to receive the record as a result of a court order or power of attorney.
- You are an attorney representing the person listed on the record.

- Birth Verification Fee: $6.00
- Birth Certificate Fee: $15.00
