Procedure

1. Fill out Form DS-11: Application For A U.S. Passport. You can download the form here: http://www.state.gov/documents/organization/212239.pdf.
2. Submit the completed Form DS-11 in person.
3. Present and submit all required documents.
4. Pay the necessary passport fee.
5. Provide one Passport Photo.

• proof of U.S. Citizenship
  • previous U.S. Passport (mutilated, altered, or damaged passports are not acceptable as evidence of U.S. citizenship.)
  • certified birth certificate issued by the city, county or state
• proof of identity
  • previous U.S. passport (mutilated, altered, or damaged passports are not acceptable as proof of identity.)
  • Naturalization Certificate
  • Current, valid
    • Driver's license
    • Government ID: city, state or federal
    • Military ID: military and dependents
• two 2x2 photos: Photo Requirements

Passport Book

• Adult (16 yrs & older) - $135
• Minors (Birth 15 yrs) - $105
• Adult Renewal - $110

Passport Card

• Adult (16 yrs & older) - $55
• Minors (Birth 15 yrs) - $40
• Adult Passport Book Holders - $30

If you apply for both documents at the same time, Cost for First Time Applicants

• Adult (16 yrs & older) - $165
• Minors (Birth 15 yrs) - $120

Validity

Explain the time until which the certificate/document is valid.
