Procedure

In Person

1. The applicants have to submit directly all the documents at the nearest Peoples Committees or Department of Justice. In case they could not submit directly, they are able to ask other people to apply for them. The authorization must be in writing and notarized or lawfully authenticated. If the authorized people are grandparents, father, mother, son, wife, husband, brother, sisters of the applicants, it does not require the authorization writing but they have to present some papers proving the relationship.
2. Filling up 1 declaration form in the prescribed form. Download here
3. The old original Death Certificate (if available) or a copy of the Death Certificate issuing before.
4. The copies with the original papers to collate or the notarized copies of following documents such as: Identity Card or Passport, Household Registration Book or Registration of Temporary Residence.
5. Written authorization or proof of personal relationships (in the case of authorization).

In Mail

In case of sending the documents by mail, all of them must be notarized copies.

• The old original Death Certificate (if available) or a copy of the Death Certificate issuing before.

Eligibility

• The parents, wife, husband, children, grandchildren or other relatives (over 18years old) of the dead can go through this procedure to obtain a death certificate of that people.
• The death has been registered in the past but the death registration book or the original death certificate has been lost, damaged or unusable shall be re-registered.
• The People's Committees of communes, wards or towns where you did register in the past could carry out the death re-registration.

Fees

• The copies of Death Certificate: 3,000VND/copy.
• Death Certificate: 10,000VND/case

Validity

The validity is ??lifetime.

Processing Time

The processing time is one working day. If the applicants submit the documents after 3:00PM, the results are given back next working day. In the case of verification, the time limit does not extend more than 03 working days.

Instructions

• Coming to the People's Committees of communes, wards or towns where you did register in the past could carry out the death re-registration.
• Filling up 1 declaration form in the prescribed form.
• In the case of re-registration at the Peoples Committees, where the applicant did not register a death before, the declaration form must be certified by the Peoples Committees, where the applicant did register a death before. Except when the applicants can present the copy of Death Certificate issued previously.
• After checking the valid documents, Justice Officials recorded in the death registration book and issue a death certificate. The chairman of People's Committee shall sign and give one original Death Certificate. The copies of the Death Certificate issued according to the request of applicants.

Need for the Document

You are legally required to obtain a death certificate. After registering a death you are able to obtain documents of death certificate for the funeral director and for dealing with the deceased's estate.

Information which might help

Death Certificates are issued only one original. The copies of Death Certificates are issued upon the request of the people applying for the birth registration.
