  • Certified Photocopy of Birth Certificate.
    • Must be inserted in the Civil Registry.
  • Photocopy of Identity Card (if greater than 09 years).
  • Photocopy of the identity card of the Father and Mother.
  • Copy of the Gazette (if parents are naturalized)
  • Copy of passport and visa (Updated)(if parents are foreigners)
  • The presence of both parents missing if any, must carry the authorization signed by the missing parent.
