Apply In Person
- To Apply for Delayed Registration (Late Entry) of Birth, the applicant has to approach the Health & Family Welfare Department, Government of Uttarakhand link “panchayat/ Directorate of Municipal Administration/Corporation / Health Department”
- An application form will be provided to the applicant by the concerned officer.
- The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
- Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
- The applicant has to attach the photocopies of the required documents along with the application form while submitting.
- Submit all the documents with the completed form to the concerned officer.
- Fee for this procedure is mentioned in the “Fees” section of this page.
- After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
- An acknowledgment receipt will be provided. Please save it for future reference.
- Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
- Once the process and the birth record verification is completed with the birth occurred hospital, the applicant will get SMS from the office for collecting the certificate.
- The applicant has to visit the centre to get the birth certificate.
- The processing time for this procedure is up to 2 to 3 weeks.

- To Apply for Delayed Registration (Late Entry) of Birth, the applicant has to approach the nearest CSC link

- An application form will be provided to the applicant by the concerned officer.

- A birth certificate in India is a permanent and official record of the child's existence.
- A child who is not registered at birth is in danger of being denied the official identity, a recognized name, and nationality. So, the applicant needs to register the birth with the respective departments.
Information which might helpEdit
- Birth event, information on which is given to the Registrar after 30 days but within one year of its occurrence, shall be registered only with the written permission of the prescribed authority and on the production of an affidavit made before a notary public or any other officer authorized in this behalf by the State Government.
