Uttar Pradesh - Online Apply Medical Certificate

Step 1 : First one has to make sure that all the required documents are in hand to apply online. If you would like to know about the documents that are required for this procedure, please visit the following link : Required Documents for Obtaining Medical Certificate

Step 2 : Please use the following link to apply online : Apply Online.

Step 3 : Now you will be taken to the login page where you have to click “For New Registration” option which is highlighted in RED in the below image.

Step 4 : Now complete the registration form with valid details and click “Register” tab which is highlighted in Yellow in the below image.

Step 5 : Now you will receive the confirmation message on the screen displaying “OTP is sent to your registered mobile no.”. Now click “OK” tab which is highlighted in RED in the below image.

Step 6 : In this page please enter the OTP in the textbox and click “Verify” tab which is highlighted in RED in the below image.

Step 7 : Now you will receive the confirmation message on the screen displaying “Mobile Verified Successfully”. Now click “OK” tab which is highlighted in RED in the below image.

Step 8 : Now you have to login into the portal using the valid credentials. Then click “Login” tab which is highlighted in RED in the below image.

Step 9 : In this page please choose “Issuance of Fitness Certificates” option which is highlighted in RED in the below image.

Step 10 : Read the instructions which appear on the screen and click the checkbox highlighted in RED. Then click “PROCEED” tab highlighted in RED.

Step 11 : Now choose “General Fitness Application” option which is highlighted in RED in the below image.

Step 12 : Now complete the application form with valid details. Make sure you have filled all the mandatory fields. In the same page you have to upload some necessary documents as a proof. To upload the files please click “UPLOAD” tab which is highlighted in Yellow in the below image.

Step 13 : Then click the checkbox highlighted in Yellow and click “PREVIEW” tab which is highlighted in Yellow in the below image.

Step 14 : Now you have to preview the details entered in the application form and click “SUBMIT” tab which is highlighted in Yellow in the below image.

Step 15 : Now you will receive the confirmation message. Please note the application number for future reference and click “OK” tab which is highlighted in Yellow in the below image to end the process.

Step 16 : Once the verification process gets completed you will be notified to download the medical certificate or fitness certificate from the portal.
