Procedure

Apply In Person:
Step 1: To Apply for a Passport Card, the applicant has to make the application to the Department of State — Bureau of Consular Affairs link
Step 2: The application can either download and complete the application form or the applicant can fill out the application form online and print it.
Step 2.1: Fill out online and Print

• To Fill out online and Print, use the following link
• Read the Privacy and Computer Fraud and Abuse Acts Notices and Disclaimers and click the checklist and click on the “Submit” button.
• The applicant has to click the “Submit” button in the “Fill out online and Print” section.
• The next page will redirect to the application form. The applicant has to fill in all the necessary details in the application form. (Incomplete application forms are not accepted).
• Once the information is completed the applicant can print the application form.

Step 2.2: Fill application manually

• To fill application form manually the applicant has to download and complete the application form by using the following link

Step 3: After filling out the application form, the applicant has to attach the necessary documents, which are mentioned in the “Required Documents” section of this page.
STEP 4: The applicant has to pay the prescribed fee that is mentioned in the Fees section of this page. The applicant has to check the fee details in the following link
Step 5: Once the above process is completed the applicant has to submit the completed application, including citizenship evidence, photocopy of ID, photo, and fees, in person at a nearby passport acceptance facility. The applicant can find their nearby passport acceptance facility in the following link either by entering their zip code or state and city link
Step 6: After submitting the application form and supporting documents the applicant tracks the application status online via the following link after 2 weeks link

Eligibility

• A U.S. citizen who intends to obtain a passport is eligible to make this application.

Fees

• Passport Book: Application Fee - $80, Execution (Acceptance) Fee - $35
• Passport Card: Application Fee - $15, Execution (Acceptance) Fee - $35
• Passport Book & Card: Application Fee - $95, Execution (Acceptance) Fee - $35

Note: The application fee and the execution fee are paid separately.

Validity

• A passport is valid for five years.

• Applicants must complete the application form before submitting it. Incomplete applications will not be accepted.
• Applicants must provide genuine information while applying for this procedure.
• The evidence must be an original or certified, physical copy. Photocopies and notarized copies are not acceptable.
• If the applicant doesn’t want to submit a photocopy of his/her citizenship evidence, he/she may submit a second certified copy of your citizenship evidence, which the authority will keep.
