ProcedureEdit

Apply online:

  1. To Apply for a Materials Permit or License, the applicant has to make the application online via link
  2. If the applicant is an existing user, they can log in directly using their “Username” and “Password”.
  3. If the applicant is a new user, they have to register first by clicking on the “Create an account” option.
  4. The applicant has to provide their email address when they create an account.
  5. While registering, the applicant has to provide all necessary information in the application form and finally click on the “Create Account” button.
  6. Once the registration is successful, the applicant can log in with their “Username” and “Password”.
  7. After logging in, the applicant has to choose the service necessary for them.
  8. The Applicant must provide all information necessary for the application procedure online.
  9. Also, upload all the necessary documents which are mentioned in the “Required Documents” section of this page.
  10. The fee for this procedure is mentioned under the “Fees” section of this page.
  11. If the application is granted, the company receives a USDOT Number immediately after submission.
  12. The FMCSA recommends that the applicant keep track of the assigned USDOT number.
  13. This entire process can be completed within 6 weeks or as per the timeline advised by the respective department authority.



Apply by Mail

  1. To Apply for a Materials Permit or License, the applicant has to approach the PIPELINE AND HAZARDOUS MATERIALS SAFETY link
  2. An application form will be provided to the applicant by the concerned officer. or it can be downloaded from the following link
  3. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  4. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  6. Submit all the documents with the completed form to the concerned office mailing address.
  7. The mailing address is mentioned in the “Office Locations & Contacts” section of this page.
  8. If the application is granted, the company receives a USDOT Number immediately after submission.
  9. The FMCSA recommends that the applicant can keep track of the assigned USDOT number.
  10. This entire process can be completed within 6 weeks or as per the timeline advised by the respective department authority.

  • All facilities or companies that treat, store, or dispose of hazardous wastes now or plan to do is eligible to make this application.



FeesEdit

  • For each registration statement submitted, all applicants must additionally include a processing fee.
  • The processing charge for each registration statement filed for registration years 2000–2001 and later is $25, and a single statement may be filed for one, two, or three registration years.
  • The processing charge for each registration statement filed for registration years 1999–2000 and prior is $50, and each statement must be filed separately for each registration year.

  • The Hazardous Materials Safety Permit HMSP Program was established by the Federal Motor Carrier Safety Administration after Congress approved a statute mandating the establishment of a permit program.
