Procedure

Once registration of death is finished, a death certificate will be issued and an extract from the state register of civil status of citizens of death for a funeral or the relevant certificate for burial, in the case of state registration of the death of the executive bodies of village, town or city (with the exception of regional significance) Board (paragraph 15 of Chapter 5 Section III of state registration of acts of civil status in Ukraine).

Eligibility

Registration of death will be filed at the request of relatives of the deceased members of the guardianship and care workers housing maintenance organizations, health care administrators, where the death occurred, and others ( part six of Article 17 of the Law of Ukraine "On state registration of acts of civil status" ). Death certificate will be issued to the deceased relatives once the death has been registered.
