Procedure

1. Fill up the appropriate form. The application shall be accompanied by copies of documents supporting those changes the voter would like to make on his/her registered personal information. There are times that the registration authority would ask to see the original document, so make sure that you have also brought those when you apply.
2. Application forms are submitted in the voter's registration office depending on the voter's address. Voters who resides outside the territory of Ukraine, must submit their applications to the relevant foreign diplomatic institutions of Ukraine, in the country they are currently residing.
3. Applications coming from abroad will be forwarded to the Register at the Ministry of Foreign Affairs of Ukraine.

Applications to change or modify your personal voter's information are submitted according to the voter's address.

A voter who is not always able to move independently, may authorize somebody to submit their application on their behalf.

Need for the Document

An electoral roll is a list of all those registered to vote, in a particular place. Article 20 of the Law of Ukraine "On the State Register of Voters" states that each voter has the right to change their personal data on the electoral roll, like in the case of newly married individuals who would like to change the surname registered on the electoral roll to reflect their new surname. This procedure may also be applied for by individuals who have changed their address.
