Ukraine - Change Your Name (Deed Poll)

An application for change of name is submitted in writing upon presentation of the passport of the citizen of Ukraine (item 3 of the Procedure for review of applications for change of the natural persons name (family name, first name, patronymic), item 3 of Chapter 5 of Section ??? of the Rules of state registration of civil status acts in Ukraine).
The application for change of name is submitted along with: the applicants birth certificate; marriage certificate (if the applicant is married); divorce certificate (in case of divorce); birth certificates of the children (if the applicant has children of tender years or underage); certificates on change of name of the applicant, father or mother if the name was previously changed; applicants photo; state duty payment receipt; (item 4 of the Procedure for review of applications for change of the natural persons name (family name, first name, patronymic), item 4 of Chapter 5 of Section ??? of the Rules of state registration of civil status acts in Ukraine).
In the course of submitting application for change of name, the authority of the state office of state registration of civil status acts compares data stated in the application with the applicants documents submitted, warns him/her of the established by law liability for untrue information provisions and the need for exchange of the passport of Ukrainian citizen (item 5 of the Procedure for review of applications for change of of the natural persons name (family name, first name, patronymic), item 5 of Chapter 5 of Section ??? of the Rules of state registration of civil status acts in Ukraine).

EligibilityEdit
Only citizens are eligible. The following rules apply:
A natural person, who achieved sixteen, has the right to change his/her family name and (or) first name at his/her sole discretion (Article 295 of the Civil Code of Ukraine).
A natural person, who achieved fourteen, has the right to change his/her family name and (or) first name upon parental consent or consent of one of parents if the second parent is dead, declared a missing person, pronounced dead, recognized as legally incompetent or of limited capability, deprived of parental rights with regard to this child, as well as if the data on the childs father (mother) is excluded from the act record on his/her birth, or if the data on the husband as childs father are entered to the act record on his/her birth upon mothers application (Article 295 of the Civil Code of Ukraine). If the guardianship is established of a natural person, who achieved fourteen, the family name and (or) the first name of such person is changed upon the guardians consent.
A natural person, who achieved fourteen, has the right to change his/her patronymic in case of change of his/her fathers first name or exclusion of the data about him as the childs father from the act record on his/her birth, and in case of his/her adoption, declaration of invalidity of the adoption or its annulment in accordance with the law (Article 295 of the Civil Code of Ukraine).

For change of the name of the natural person, who achieved fourteen, a relevant application is submitted along with the birth certificate, a notice from the place of residence and a written statement of parents (one of parents - if the second parent is dead, declared a missing person, pronounced dead, recognized as legally incompetent or of limited capability, deprived of parental rights with regard to this child, as well as if the data on the childs father (mother) are excluded from the act record on his/her birth, or if the data on the husband as childs father are entered to the act record on his/her birth upon mothers application) (item 3 of the Procedure for review of applications for change of the natural persons name (family name, first name, patronymic), item 3 of Chapter 5 of Section ??? of the Rules of state registration of civil status acts in Ukraine).
A statement of parental consent (one of parents) or of guardians consent for the indicated persons change of name may be submitted by them personally, and if the statement, due to some valid excuse, cannot be submitted personally by one of parents, such notarized or equated statement may be submitted by the second parent on his behalf (item 3 of the Procedure for review of applications for change of the natural persons name (family name, first name, patronymic), item 1 of Chapter 5 of Section ??? of the Rules of state registration of civil status acts in Ukraine).
A natural person, who achieved sixteen, but did not get the passport of Ukrainian citizen, present the birth certificate and a notice from the place of residence provided that less than one month passed of the date of this age achievement (item 3 of the Procedure for review of applications for change of the natural persons name (family name, first name, patronymic), item 3 of Chapter 5 of Section ??? of the Rules of state registration of civil status acts in Ukraine).

The name of the natural person, who is the citizen of Ukraine, consists of the family name, first name and patronymic unless otherwise provided by the law or tradition of the national minority such person belongs to (Article 28 of the Civil Code of Ukraine).
If you are using, or wish to use, a name other than your registered name, there is no legal requirement that you formally change your name. It is, however, recommended that you change your name officially, both for identification and as evidence of your name change.
Name change generally refers to the legal act by a person of adopting a name different from their name at birth, marriage or adoption. The procedures and ease of a name change vary between jurisdictions.
