Procedure

1. To replace your lost or stolen passport, you must first file a police report about the incident. You will need to submit the report when you apply for a replacement passport.
2. Complete the Lost or Stolen Passport Notification form and BOT Passport Application Form. Download links are provided under "Documents to Use" section. You may also obtain the form in person in these following offices:
   1. at the Ministry of Border Control and Employment One Stop Shop Customer Service Centres located at Sams Building, Providenciales and Three Degrees Office Complex, Grand Turk
   2. at the Immigration Department Offices or District Commissioners offices on the island of South Caicos and North Caicos
   3. at the District Commissioners Office on Salt Cay
3. Submit your fully completed application form along with the required photographs and supporting documents at any of the following locations:
   1. at the Ministry of Border Control and Employments One Stop Shop Customer Service Centres located at Sams Building, Providenciales and Three Degrees Office Complex, Grand Turk
   2. at the Immigration Department Offices or District Commissioners offices on the island of South Caicos and North Caicos
   3. at the District Commissioners Office on Salt Cay
4. Pay the passport fees.
5. You will be contacted if additional documents are needed or if it is necessary to conduct an interview. If none of the above are required, your application will be case-worked and you will be contacted when the passport is available for collection.
