Here are the procedures in applying for a Disability Identification Card in Thailand:

  1. First you must prepare all the necessary documents needed when you apply for the card. A list is provided below for your reference. Make sure that you have all the required documents before beginning the application process to avoid delays.
  2. One document that you will need to submit along with your application is medical certificate issued by a medical professional. Government hospitals can issue disability credentials all over the country. You may also obtain one from a private hospital, a list of private hospitals that offers this type of service is provided below under 'Information which might help' section.
  3. Once you have all the requirements you may submit your application in these following locations:
    1. Bangkok
      (1) Department of Social Work, Rehabilitation Medicine Siriraj Hospital
      (2) Social Work Department Queen Sirikit National Institute of Child Health
      (3) Phramongkutklao Hospital
    2. Provinces
      (1) Provincial / District Hospital Which has a comprehensive service center in a hospital
      (2) a local government organization
      (3) or another government agency as announced by the provincial governor
  4. The Department of Promotion and Development of the Quality of Life of the Disabled will review your application and will inform you when the identity card will be released. You can also find out more about the application procedure here - link.

Required DocumentsEdit

Required documents for the disabled:

  • You will need to submit a copy of your identification card
  • Certified copy of birth certificate - for persons under the age of fifteen
  • ID photo of applicant taken not more than 6 months ago
  • Disability certification documents which is certified by a medical professional of a public hospital or private hospitals prescribed by the Director-General.
  • If the condition of the applicant is evident, the statement of the officer receiving receiving the application for the disability status transcript will be used as evidence.


In case the disabled is unable to come to the office:

  • Certificate of being a caregiver to the person with disabilities
    • Guarantor: Headman, headman, civil servant, government official Employees Local council members, community chairmen, permanent employees (Government agencies, state enterprises) in which the certifier must reside or perform the duties in the same area where the disabled person currently resides.
  • Copy of the disabled applicant's identity card (original must also be presented)
  • A copy of the certifier's identity card (original must also be presented)
  • If the application is to be submitted by someone on behalf of the person with disabilities
    • A copy of the identification card of the person acting on behalf
    • A power of attorney Is evidence that it has been given by a disabled person (The witness must sign completely.)

The Department of Promotion and Development of the Quality of Life of the Disabled is in charge of issuing identification cards to people with disabilities.

Need for the DocumentEdit

The Department states that people with disabilities have the right to receive public facilities. As well as welfare and other assistance from the state for rehabilitation and development so they can live their lives to the fullest. The government also offers tax deduction to those who are caring for disabled persons.

To be able to avail such assistance and benefits you may be asked to present evidence of your situation and will be asked to show a Disability Identification Card.
