Procedure

Apply through Meeseva

1. Application to change your date of birth in the birth certificate can be collected from Meeseva centre.
2. Application for Agricultural Income certificate can be collected from Meeseva centre. Following link can provide you information related to nearest Meeseva Centres: Meeseva centre link. Click on the “Contact Us” option.
3. Fill the application form and attach all the required documents.
4. Go to your Meeseva centre. Submit the completed application enclosing entire required document as per our “Required Documents” section and submit it along with the fees as applicable. Meeseva operator will do the online process.
5. The applicant will receive an application number as acknowledgement. The applicant will also get transaction number as text message on his registered mobile number.
6. The online details will be processed by respective department.
7. The important factor to check the record from the Medical Institution, who conducted delivery will be verified.
8. If the record is available, and it has the correct date as required by applicant to change in birth certificate then the work is easy.
9. If the date in the Medical Institution record is the same as in the birth certificate, then the applicant will be informed to get the letter from the Medical Institution, who conducted delivery supporting the requirement as guided by authorities.
10. On decision of the respective department authority, applicant will get his or her Certificate from Meeseva centre by producing the acknowledgement.

• Application form
• Valid birth certificate
• Affidavit from local notary
• Address proof (Voter ID, Driving Licence etc.) of parents as per requirement
• Aadhaar card
• Education Certificates as required
• Aadhaar Card of Parents as per requirement
• Report from Medical Institution who conducted delivery. (As per requirement)
• Affidavit from two credible persons. (As per requirement)

• All original and copy of original with self-attestation as advised by respective authorities to be submitted as per requirement.
