Apply In Person

  1. To Apply for a Family Planning certificate in India, the applicant has to approach the Ministry of Health and Family Welfare or Visit Government health centers and hospitals. link
  2. An application form will be provided to the applicant, after consulting with a doctor or a health worker for the process of applying for a family planning certificate.
    Note: They will also provide the applicant with information on different family planning methods and help the applicant choose the one that suits them best.
  3. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  4. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  6. Submit all the documents with the completed form to the concerned officer.
  7. The applicant has to pay the prescribed fee as per mentioned by the concerned officer.
  8. After submitting the application form, the applicant will receive a confirmation message with a reference number.
  9. After submitting the form, the applicant will be asked to choose a family planning method.
  10. The applicant can choose from various options, including contraceptive pills, intrauterine devices (IUDs), condoms, and sterilization.
  11. The applicant has to attend counseling and education on the benefits, risks, and side effects of the chosen method. Counseling and education will also cover how to use the method effectively.
  12. The concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  13. An acknowledgment receipt will be provided. Please save it for future reference.
  14. Once the applicant has chosen the method of family planning and attended the council.
  15. For further process, the applicant has to follow the instruction given by the officer.
  16. Once all the process gets completed successfully, the concerned authority will issue a Family Planning certificate.
  17. This entire process can be completed within 30-40 days or as per the timeline advised by the respective department authority.
