Apply In Person:
- To Apply for Family Planning Indemnity Scheme (FPIS), the applicant has to approach the Ministry of Health and Family Welfare link
- An application form will be provided to the applicant by the concerned officer or it can be downloaded from the following link check the form from page no 33.
- The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
- Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
- Submit all the documents with the completed form to the concerned officer.
- After submitting the application form, the applicant will receive a confirmation message with a reference number.
- The concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
- There is no fee for Family Planning Indemnity Scheme (FPIS).
- An acknowledgment receipt will be provided. Please save it for future reference.
- Then the application will be processed further.
- Once the application got approved, the applicant will receive a call or notification through SMS.
- Once the process gets completed, the concerned officer will issue a certificate of coverage.
- This entire process can be completed within 30-40 days or as per the timeline advised by the respective department authority.

- ID proof (Aadhar card, Voter ID, Passport)
- Address proof – (Driving license, passbook, utility bill, etc)
- Discharge summary or certificate from the hospital or clinic where the sterilization procedure was performed.
- Sterilization consent form
- Sterilization operation report
- Any other document requested by the concerned officer

EligibilityEdit
- The applicant must have Indian citizenship.
- The applicant must have undergone a sterilization procedure in a government-approved hospital or clinic.
- The applicant must be between the age of 18 to 60 years.
- The applicant must not have any pre-existing medical conditions that may lead to complications after the sterilization procedure.
