Apply In-Person:

  1. Application form for obtaining an Industrial Hygiene Inspection Certificate is available in the Occupational Safety and Health Authority (OSHA) contact link
  2. Applicant must provide all information in the application form and submit it to concerned authorities.
  3. Applicant should make sure to carry photocopies and originals of all documents that are mentioned under “Required Documents” section of this page.
  4. Applicant must book an appointment at the OSHA and on the date of appointment, the OSHA will send concerned authorities for conducting an inspection in front of the applicant or any other company officials.
  5. The inspection officer will conduct an inspection and provide the applicant with an OSHA government bill for fee payment process.
  6. Applicant will have to go to the National Microfinance bank contact link for payment of their fee.
  7. After successful payment, the applicant will be provided with a receipt which will act as a proof of payment.
  8. Fee for this procedure is TZS 1,920,000/-.
  9. Applicant has to come back to the OSHA office for submitting the receipt of application payment.
  10. If all above-mentioned procedures were followed properly by the applicant, concerned authorities will notify the applicant regarding the availability of Inspection Certificate.
  11. After getting the notification, applicant can visit the office for getting their certificate.
  12. The processing time for this procedure is 15 days.
  13. If there is no information regarding the certificate is provided by concerned authorities, applicant can visit the office to know status of their application.
