Apply In-Person:
- To apply for optometry facility or permit permit or license, the applicant has to visit the Ministry of Health, Social Welfare, Elderly, Gender, and Children contact link
- Applicant should provide all the information that is requested in the application form.
- Applicant should make sure to carry both photocopies and originals of all the documents that are needed for completing this procedure.
- Documents that are needed for completing this procedure are mentioned under the “Required Documents” section of this page.
- Applicant must attach both photocopies and originals of documents (whichever is necessary) along with the application form before submitting it at the office.
- Fee for this procedure has to be paid as per the authority’s request.
- If all these procedures were followed properly by the applicant, the optometry facility or permit license will be provided within 10 days.
- If there is no information regarding the license is provided within 10 days, the applicant can visit the office to know the status of their application.
