Procedure

• Obtain the application from any nearer Divisional Secretariat.
• Complete the application and submit to the Divisional Secretariat of the area in which the place of birth is occurred.
• Affix the required fee with the application form by stamps.
• If you like to obtain the birth certificate by post, submit a stamped envelop with the application.

Fees

• If the number of the certificate of birth & the registered date is known Rs.25.00
• If don't know the number of the certificate of birth or the registered date,
  • for search of birth registers for period of 03 months Rs.50.00
  • for search of birth registers for period of 02 years Rs.100.00

Instructions

• Refer the application to the Divisional Secretariat of the area in which the place of birth is occurred.
• Application can be obtained from any Divisional Secretariat.
• Required fee should be affixed in the application form by stamps.
• Duly filled application should be submit to the Divisional Secretariat.
• If you like to obtain the birth certificate by post, submit a stamped envelop with the application.
• If the birth is registered a birth certificate will be send and if the birth is not registered a letter to the effect will be send.

• No. Copies required

• Registrars Division
• Number and Date of Registration entry (if known)
• Period of search desired

It is an official form that gives details on the time and place of a person's birth, and his or her name, sex, mother's name and (usually) father's name. The word "birth certificate" can refer to either the original document or a certified true copy of or representation of the original record of birth document.
