Procedure

1. Gather all documents that you will need to present or submit when you apply for the procedure (see list below).
2. Get in touch with the office of the District Registrar of the Divisional Secretariat for further information. You may use the contact information provided under "Office Locations & Contacts" section to inquire where you can submit your application.

Required Documents

Documents to be submitted by a resident child of a children's home approved by the government:

• Affidavit of the matron of the children's home
• Medical certificate issued by a government medical officer determining the age of the child *Other documents containing information of the child (if any)

Documents to be submitted by a child under the guardianship of persons other than a government approved children's home:

• Affidavit of the guardian
• Proof of guardianship :
  • Grama Niladhari certificate or
  • Certificate of the probation and Childcare Service Officer
  • Certificate from the Social Services Department
  • Certificate issued by the Inspector of Police
• Medical certificate issued by a government medical officer determining the age of the child *Health Development Record
• Copy of the school admission register if the child attends school

Eligibility

Certificate of Probable Age is issued to a:

• resident child of a children's home approved by the government.
• child under the guardianship of persons other than a government approved children's home.
