Procedure

Apply In-Person:

1. To Obtain Proof of Age Card, the applicant has to Service SA customer service centre. Contact details are mentioned in “Office Locations & Contacts” of this page. Or use the following given Contact Link
2. An application form is available at the following Link
3. The applicant has to get the form in Access Canberra Service Centre and fill-in all the details in the form.
4. Applicant should make sure to attach all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
5. Submit the application with the required document to appropriate office.
6. Submitting address is mentioned in “Office Locations & Contacts” of this page.
7. The fee of this procedure is mentioned under the “Fees” section of this page.
8. Proof of age card processed within 3 weeks.
9. Government of South Australia will issue the certificate and notice the applicant via post.

2. If the applicant is an existing user, they can login directly using their “Email” and “Password” by clicking “Log-in” option in the given page.
3. If the applicant is a new user, they have to register first by clicking on the “Create account instead” option.
4. An applicant has to provide their own email address when they create an account.
5. While registering, applicant has to provide all necessary information in the application form and finally click on “Create an account” button.
6. Once the registration is successful, the applicant can login with their “Email” and “Password”.
7. After logging in, the applicant has to choose the service necessary for them.
8. The Applicant must provide all information necessary for the application procedure online.
