South Australia - Apply for Proof of Age Card
ProcedureEdit
- Step 1: Complete the application form for proof of age card.
- Step 2: Return it to a Service SA customer service centre, with the following:
- Evidence of identity - a minimum of three documents that verify your identity, age, signature and residential address. If you have a photographic learner's permit or licence you may not need to supply full evidence of identity.
- Payment.
- Allow up to a month for your proof of age card to arrive in the mail.
Required DocumentsEdit
- Evidence of your identity is required when applying for a learners permit, driver's licence, or registering a vehicle for the first time.
- At least three documents are required to provide evidence of identity.
- At least one document must be provided from both category A (evidence of existence) and category B (evidence of use of identity). Your signature, date of birth and current address must be on at least one piece of evidence.
- If none of the documents from category A or B shows your current address, you must provide a fourth document showing your current residential address.
- Documents must be originals and issued within the allowable time. Photocopies, extracts or commemorative certificates are not acceptable.
Category A - evidence of existence
- The following documents are acceptable:
- an Australian full birth certificate showing parental details (a Commemorative Certificate, certified copy or extract is not acceptable);
- an overseas birth certificate showing parental details provided a passport or an official Australian travel document is also shown;
- a current Australian passport or one that has expired within the last two years;
- a current overseas passport (with or without visa) or an overseas passport that has expired within the last two years (provided it is accompanied by a current Australian visa);
- Note: Electronic visas are not acceptable as Evidence of Identity
- an Australian naturalisation or citizenship document or immigration papers issued by the Department of Immigration and Citizenship (DIAC). Note: A person under the age of 18 who is naturalised along with their parents may not necessarily be issued with an individual citizenship document; they may instead appear on the parents citizenship papers;
- Department of Immigration and Citizenship (DIAC) Certificate of Evidence of Residence Status";
- Department of Immigration and Citizenship (DIAC) travel document (valid up to five years after issue). Note: A person under the age of 18 who immigrates to Australia with his/her parent(s) will generally be included on the immigration paperwork of their parent(s);
- community detention: letter of introduction, issued by the Department of Immigration and Citizenship (DIAC) or the Australian Passport Office;
- an Extract from Register of Citizenship by Descent (issued by an Australian embassy where the parent(s) are Australian-born). Note: the term Extract is the official title and is not an extract in the same sense as an extract from an Australian full birth certificate;
- photograph and/or signature stored on the photographic licence database;
- South Australian or Federal Police Officer photo identity card;
- A current (or expired by up to two years) Roads and Maritime (formerly Roads and Traffic Authority) issued New South Wales Photo Card. (Note: NSW issued Proof of Age cards are not acceptable as Evidence of Identity);
- South Australian proof of age card (issued after 9 October 2006);
- Australian photo drivers licence or learners permit (current or expired up to two years). The licence / permit must display a card number and not be reported as lost, stolen or destroyed.
Category B evidence of use of identity
- The following documents are acceptable:
- a current Medicare card, Pensioner Concession Card, Department of Veterans' Affairs entitlement card or any other current entitlement card issued by the Australian (i.e. Federal) Government
- a telephone, gas or electricity bill up to six months old
- a water rates, council rates or land valuation notice up to twelve months old
- Australian Defence Force photo identity card (excluding civilian staff and family)
- an electoral enrolment card or other evidence of enrolment up to two years old
- student identity document from an educational institution with photograph and/or signature (current or expired up to twelve months)
- Australian issued security guard or crowd controller licence showing photograph
- occupational licence (OCBA licence) showing photograph
- an armed services discharge document up to twelve months old
- Australian issued firearms licence showing photograph
- a current credit card, that shows the embossed name and signature, or account card from a bank, building society or credit union, or a passbook or account statement up to one year old.
Note: Screen dumps from financial institutions are not acceptable, with or without financial institution branch stamps.
Evidence of residential address
- A person must be a resident of South Australia to be eligible for the issue of a learners permit or drivers licence. Evidence of a South Australian address is required when a person applies for a drivers licence or learners permit.
- A person is only required to supply one of the documents listed below if their residential address has not been established by a Category A or B document:
- current South Australian vehicle registration certificate
- South Australian drivers licence or vehicle registration renewal for the coming period
- a contract of sale, lease arrangement or rental receipt for his/her place of residence up to two years old
- last or current financial year Australian Taxation Office tax assessment (note: client must have blanked out their Tax File Number (TFN) prior to presentation)
- certificate or statement of enrolment from an educational institution (up to one year old)
- a letter from the principal of a recognised educational institution verifying the persons address (up to one year old)
- a statement or letter from a bank, building society or credit union (up to one year old) which verifies the persons residential address.
Note: Screen dumps from financial institutions are not acceptable, with or without financial institution branch stamps.
