Apply In-Person:
- To Apply for Aged Care Assessment Program, the applicant has to approach the Department of Health and Aged Care link
- Initiate the application process by contacting My Aged Care, the government portal that handles aged care services. You can reach them by calling the My Aged Care contact centre at 1800 200 422.
- Inform the My Aged Care representative that you would like to apply for the ACAP assessment. They will guide you through the process and provide the necessary information.
- An application form will be provided to the applicant by the concerned officer.
- The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
- Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
- The applicant has to attach the photocopies of the required documents along with the application form while submitting.
- Submit all the documents with the completed form to the concerned officer.
- Based on your care needs, My Aged Care will arrange a time for an assessment. This can be conducted over the phone or in person, depending on your circumstances and preferences.
- There is no fee for Aged Care Assessment Program.
- On the scheduled date, be prepared to provide relevant information about your health, daily activities, and any support you currently receive. The assessor will evaluate your care needs and determine your eligibility for aged care services.
- After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
- Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
- Once the process gets completed, the applicant gets a call from the office regarding the application.
- After the assessment, you will receive a letter or notification with the outcome of your assessment. This will include information on the level of care you are eligible for, such as Home Care Packages or residential care.
- If you are deemed eligible for aged care services, you can then explore and discuss your care options with My Aged Care. They will provide guidance on available services, providers, and the process for organizing care.
- This entire process can be completed within 2 – 3 weeks or as per the timeline advised by the respective department authority.
