Procedure

Apply In-person

1. To apply for a Replacement for lost, damaged or stolen Passport in person, the person who wants to apply must visit any nearest office of Department of Home Affairs in South Africa.

3. Go to the office of DHA and obtain the application form to apply for a Passport or Travel Document (BI-73) from the concerned Department. Form BI-73 can also be downloaded by clicking the following document: Link
4. Also you have to complete a form BI-335 for Reporting the loss of Passport which can be obtained from the office of DHA.
5. Complete the application form as required with relevant and appropriate details. Specify the type of passport or travel document that you want under the “Application for” heading, by marking with an “X”.
6. Proceed filling the application with information relevant to your selection. Make sure that you have all the documents listed under the “Required documents” section of this page.
7. Once completed filling the application forms, submit them along with all the required documents at the respective department in the DHA office.
8. After submission, the applicant will be required to be present at the office for further verification.
9. The passport officer will verify your identity with the identification document provided. Then, he will check that the photograph submitted is a true image of the applicant.
10. Then, the fingerprints of applicant will be recorded and crosschecked with that in the National Population Register. In the meantime, you will be directed to pay the prescribed fees.
11. After all the verification processes have been completed and the passport officer is satisfied with the applicant documents’ provisions, he/she will make the further proceedings for the issuance of new Passport or travel document.
12. The document is sent to the postal address of the applicant in most cases. Or would be asked to collect at the office with prior intimation.

Eligibility

• Any person, who had a passport previously and lost it by any means, can apply for a new Passport.
• The person applying for a Passport or Travel document must possess a valid South African Identity document in case of SA citizens and a permanent residence permit and an original SA Non-citizen Identity document in case of non-citizens.

Instructions

• Please be aware that the applicant must be present at the office of DHA at the time of application submission, for approval of the passport or travel document request. If the application is for a minor, then both of the parents must be present at the time of submission.
• In case if you do not have a valid South African Identity document, it is essential that you should apply for a birth certificate or any South African Identity document at the first place and then proceed with applying for a travel document or passport.
• If you are in need to travel immediately, you can apply for an Emergency travel document/certificate by the same format.
• In case of emergency travel document/certificate, remember that particular countries do not have such provisions. Please check whether it is permitted in the country for which you want emergency travel certificate for, with the concerned authorities of that country.
