South Africa - How To Apply for alteration on your matric certificate (changing your name or surname)

Apply In-Person:

  1. To apply for any alteration on your Matriculation certificate in person, the person whose certificate is to be altered should visit the office of Department of Basic education or office of any Provincial Education Department.
  2. The contact details and address information of the DBE can be found in the following link:Link Details of PED offices are also available under the “Provincial Offices” section in the above link page.
  3. Visit the office of any of the above mentioned departments and get the Application for Change/Correction of Personal Particulars from the respective authority/counter as directed.
  4. Fill the application form as applicable to your requirement and relevant information. Make sure that you have all the documents listed in the “Required documents” section of this page.
  5. Attach all the documents with the completed Application form and submit it to the respective department at the office of any of the above mentioned departments. And pay the required amount of fees as directed, at the office.
  6. After submission, the application form and attached documents will be verified and validated correspondingly by the concerned officials.
  7. When the officials find the application and documents in order, they will amend necessary proceedings for the re-issue of the Matriculation Certificate.
  8. Once the Matriculation Certificate is all ready to be re-issued, it will be issued to the applicant through the means that he/she had mentioned in the application form.



Apply through post

  1. To apply for any alteration on your Matriculation certificate through post, the person whose certificate is to be altered should submit the application to the office of Department of Basic education or office of any Provincial Education Department by post.

  4. Fill the application form as applicable to your requirement and relevant information. Make sure that you have all the documents listed in the “Required documents” section of this page.
  5. Attach all the documents with the completed Application form and submit it to the respective department at the office of any of the above mentioned departments through post.
  6. Also attach the fees by postal order or cheque along with the application package.
  7. After submission, the application form and attached documents will be verified and validated correspondingly by the concerned officials.
  8. When the officials find the application and documents in order, they will amend necessary proceedings for the re-issue of the Matriculation Certificate.
  9. Once the Matriculation Certificate is all ready to be re-issued, it will be issued to the applicant through the means that he/she had mentioned in the application form.

  • Original national/senior certificate and a certified copy
  • Affidavit or confirmation statement by the candidate
  • Detailed affidavit from SAPS
  • The original Certificate
  • Confirmation letter from DHA witnessing the alteration request
  • Any certificate that bears the applicant’s name and DOB. E.g. Birth certificate, clinic card, school report.

EligibilityEdit

  • Any person, who wants to alter any particulars in his /her matriculation certificate, can apply providing necessary proofs for the alteration.
  • In case of alteration, a new Certificate will come into amendment only when the old one is submitted to the office.
  • The person applying must possess both old and new Identity documents.

InstructionsEdit

  • Within the six-month period after certification (first issue), but not later than a year after the last examination, a request for a re-issue will be considered.
  • A change to an original certificate will not be considered unless the original certificate (not replacement) is returned.
  • You must go to the nearest DBE/PED offices in person so that DBE/PED officials can interview you before they make the amendments.
