Procedure

In person

1. You may send your application to obtain a replacement card person at a HIIS regional office or service counter. You can also authorised somebody else to process your request.
2. If the reason why you are requesting a new card is because your current card's chip is malfunctioning, then you must bring it to your nearest regional unit or HIIS branch to confirm the malfunction.
   If you have changed your name or surname you will also need to surrender your old card for cancellation before a new one is issued.
3. Replacement fee:
   1. Lost/stolen/damaged card: You will need pay for the new health insurance card. If the card you are replacing is (was) more than 10 years old, you will get a new card free of charge.
   2. Change of data on the card: The issue of a new card is free of charge.

• Lost / stolen / damaged cards: You may request for a new insurance card in case you have lost your card or it has been stolen or the chip does not work anymore or damaged.
• Change of data on the card: You will also need a new card if you change your name or surname, since this information is displayed on card.
