Slovakia - Replace Lost / Stolen eID (Electronic ID) Card

ProcedureEdit

  1. In case of loss or theft of your eID card, you must immediately go to the nearest District Police Directorate and report the incident. Before you file your report, make sure that your eid is really lost, because once a card is reported missing it will be automatically cancelled. Note: A link is provided under "Office Locations & Contacts" section which you can use to get in touch with the nearest District Police Directorate.
  2. To apply for a new card, you can go to any Distric Police Directorate.
  3. Fill up the application form.
  4. Submit the form along with all other required documents and pay the replacement fee.
  5. Ask the processing officer when can you can come back to collect your card.
  6. Once the ID card is issued to the applicant, he/she must confirm receipt of the said document by affixing his/her signature on the receiving report upon collecting the card. ID cards may be picked up by another as long as he/she can present an ID card and a power of attorney to represent the verified signature.
