ProcedureEdit

  • Visit any Slovak Registry local issuance office in your area of residence.
  • Fill in all sections of the marriage certificate application form.
  • Include proof of eligibility
  • Attach your identity documents
  • Pay the application fee
  • Pay the urgent fee (if required)
  • Pay the return postage fee (if required)

For service by mail, fax or email to the Slovak Registry offices. The Birth Certificate Application must be signed and dated in front of a notary public. Payment is accepted by credit card (MasterCard, VISA, or Discover), check or money order and is deposited upon receipt.

InstructionsEdit

If the applicant is not a spouse, a judicial or other proper purpose must be documented. An example of a judicial or other proper purpose would be a marriage record needed by the applicant to claim a benefit. Documentation would consist of an official letter from the agency verifying that to process the claim they require from the applicant a copy of the requested marriage record.


Required InformationEdit

  • Day, month and year of marriage, if the marriage took place outside the seat of the registry office, place of marriage,
  • Name, maiden name, date, month, year and place of birth, social security number and the status of men and women who were married (hereinafter referred to as "spouses")
  • Name, maiden name, date, month, year and place of birth of the couple's parents
  • Agreement of the spouses of the name
  • Agreement on the name of their common children; agreed surname indicating the male and female form
  • Nationality of the spouses
  • Names, surnames and social security numbers of witnesses
  • Day, month and year of registration



Need for the DocumentEdit

A marriage certificate is an official recorded document issued by a governmental authority that proves that the couple listed on the marriage certificate have a legal marriage. In most areas, a marriage record is part of the public record.

A marriage certificate usually contains who married who, when they were married, where they were married, who married them, and who was there.



Information which might helpEdit

It is generally the responsibility of the officiant of your wedding ceremony to make sure that your signed marriage license is delivered to the registry office. Since the registry office has to process the filing and recording of the marriage license, you usually have to wait several weeks before receiving your marriage certificate.


Other uses of the Document/CertificateEdit

A marriage certificate is required to:

  • prove your marriage is legal
  • ensure that the rights of spouses and children are respected
  • apply for certain social benefits
  • settle an estate.

  • In some locales, once a marriage license is signed and filed with the county clerk, the license becomes the marriage certificate.
  • Generally, in order to receive an additional copy of a marriage certificate, you need to send your request along with the required fee to the County Clerk/Recorder's Office of the county where the marriage took place.
