Procedure

Apply In-Person:

1. To Report a Lost Passport, the applicant has to approach the passport unit of Immigration and check point authority contact link or the applicant can also approach Ministry of Foreign Affairs link
2. Applicant has to collect the application form for report a lost passport from the authorised officer.
3. Applicant has to fill-in all the necessary details in the application form.
4. Make sure that you have all the documents mentioned in the “Required Documents” section of this page.
5. Submit the application form to the respective office.
6. Fee for this procedure is given in the “Fees” section of this page.
7. Upon successful submission the report filed by the authority officer.

Apply online

1. To apply online the applicant has to visit Immigration and Checkpoint authority website’s loss of passport page or tap the following link
2. Under” I WANT” Choose the services you require. i.e. (Self or on behalf of others in case of a minor or disable/elderly/sickly persons)
3. The applicant will be redirected to the next page, The applicant has to login with either Singpass app or Singpass Id number and password

   Note: If you don’t not have an account you can register by clicking on register for singpass, read the information marked 1 and 2 and agree to the terms and condition and click on register now, no the next page entry your personal information and submit. You will be issued with you Sing Pass ID number and password

4. On the next page read through the instruction carefully and fill the applicant form correctly, please verify that all the information is correct
5. Upload the required documents that are mentioned in the “Required Documents” section of this page.
6. Pay the required fee using your credit/debits card and click submit.
7. After successful payment the report of loss of passport is filed.

• Original IC (for adults) or birth certificate (for applicants aged below 16*)
• Details on how the passport was lost
• Police report (if the applicant is a victim of crime, e.g. Theft)

Eligibility

• Any person who lost their passport are eligible to make this application.

Fees

• For apply in online the applicant needs to pay the non-refundable fees in addition to the prevailing passport fee of S$70
• For apply in person the applicant needs to pay the non-refundable fees in addition to the prevailing passport fee of S$80
• The applicant needs to pay S$50 (for the first passport loss)

Validity

• The validity of the report is forever.

• A loss of passport report is necessary it is proof to apply a new passport.
