Procedure

Apply In-Person:

1. To Report a Loss of Identity card (IC), the applicant has to approach the passport unit of Immigration and check point authority contact link or the applicant can also approach Ministry of Foreign Affairs link
2. Applicant has to collect the application form for Loss of identity card from the authorised officer.
3. Applicant has to fill-in all the necessary details in the application form.
4. Make sure that you have all the documents mentioned in the “Required Documents” section of this page.
5. Submit the application form along with the required document to the respective officer.
6. Fee for this procedure is given in the “Fees” section of this page.
7. Upon successful submission the report filed by the authority officer.

Required Documents

For Singapore citizens, the applicant will need to provide the following documents:

• A police report (if he/she is a victim of crime, e.g. theft) or make a declaration at ICA Building (for any other reasons)
• Baptism or religious certificate (if the applicant wants to include a religious name)
• Deed poll for a change of name (if applicable)

Note: Consent from all parents / legal guardians is required for the change of name if the registrant is below the age of 21

• Marriage certificate (if the applicant wants to include your married name)
• Recent passport-sized, hardcopy, colour photograph taken within the last three months. (Click here for information on photo requirements)

For Singapore permanent residents, the applicant will need to provide the following documents:

• A police report (if you are a victim of crime, e.g. theft) or make a declaration at ICA Building (for any other reasons)
• Image of the pages of the applicant’s current passport which contains personal particulars
• Recent passport-sized, hardcopy, colour photograph taken within the last three months.

Eligibility

• Any person who lost their Identity card are eligible to make this application.

Fees

• For lost identity card You will need to pay the fees of $100 for first loss and $300 for second and subsequent losses.

Validity

• The report is valid for until the new identity card is ready for collection.

Processing Time

• The processing time for the replacement of an IC is one month. Upon receipt of payment, the applicant will be issued with a collection slip providing the date from which the new replacement IC will be ready for collection.

• Applicant must complete the application form before submitting. Incomplete applications will not be accepted.
• Applicant must provide genuine information while applying for this procedure.
• If he/she subsequently found your lost IC after receiving a replacement, they must return the found IC to ICA.
