Procedure

Apply In-Person:

1. To Get Notification of Infectious Diseases, the applicant has to approach the Ministry of health link
2. The applicant has to download and complete the application form given in the following link
3. An application form will be provided to the applicant from the respective officer.
4. The applicant has to complete all necessary details in the application form.
5. After filling the application form, make sure to attach all the documents listed in the “Required documents” section of this page.
6. The applicant has to pay the prescribed fee as per the authority's request.
7. Submit the application form to the ministry of health. contact details of the office were given in the “Office Locations & Contacts” section of this page.
8. The authority will review the application and contact the applicant via email or SMS.
