Apply In-Person:

1. To Get Financial Assistance under Community Health Assist Scheme (CHAS), the applicant has to approach the Ministry of health link
2. The applicant has to download and complete the application form given in the following link
3. The applicant has to complete all necessary details in the application form.
4. After filling the application form, make sure to attach all the documents listed in the “Required documents” section of this page.
5. The applicant has to pay the prescribed fee as per the authority's request.

7. The authority will review the application and contact the applicant via email or SMS.
