Procedure

Apply In Person

1. To Get Developmental Disability Registry (DDR) Identity Card, the applicant has to approach the National Council Of Social Service link
2. Applicant has to get the application form from the respective office or download the form by using this link.
3. Applicant has to fill all the necessary details in the application form.
4. Make sure that you have all the needed documents mentioned in the “Required Documents” section of this page.
5. After completing the form, submit the completed application form along with the necessary documents to the respective authority.
6. Once the above process gets over, the authority will verify the documents.
7. Once it is processed, and the card is ready, the applicant will receive an SMS or call on the registered mobile number to collect it.
8. Once the process gets completed, the applicant can get their permit from the officer by visiting the office.
9. Fee for this procedure has to be paid as per the authority’s request.
10. The processing time for this procedure is within 30 days.
