Procedure

FOR CREMATION

1. For cremation, the applicant has to contact the Mandai Crematorium and Columbarium Complex Link
2. Applicant can book through call or can visit the office.
3. Before booking, the applicant has to read the rules and regulations/ terms and conditions using the following Link
4. Bookings for cremation can be made over the phone, online, or in person at the booking office.
5. Applicant has to give the letter or get the form from the office.
6. Applicant has to fill-in all the details in the form.
7. Applicant has to attach the documents while submitting the form, is mentioned in the “Required Documents” section of this page.
8. The payment is to be made at the booking office before the cremation, and the details is given in the “Fees” section of this page.

FOR BURIAL

1. For burial, the applicant has to contact the Choa Chu Kang Cemetery Office Link
2. Applicant can book through call or can visit the office.
3. Before booking, the applicant has to read the rules and regulations/ terms and conditions using the following Link
4. Applicant has to give the letter or get the form from the office.
5. Applicant has to fill-in all the details in the form.
6. Applicant has to attach the documents while submitting the form, is mentioned in the “Required Documents” section of this page.
7. The payment is to be made at the booking office before the burial, and the details is given in the “Fees” section of this page.

FOR COLUMBARIUM SERVICES

1. For columbarium services, the applicant has to contact Choa Chu Kang Cemetery Office Link or Choa Chu Kang Cemetery Office Link
2. Applicant can book through call or can visit the office.
3. Before booking, the applicant has to read the rules and regulations/ terms and conditions can use the following Link
4. Applicant has to give the letter or get the form from the office.

6. Applicant has to attach the documents while submitting the form, is mentioned in the “Required Documents” section of this page.
7. The payment is to be made at the booking office before the ash scattering service.

• Identification Papers of Next-of-kin and Applicant (e.g. NRIC, passport, etc.)
• Letter of Authorisation (if applicant is not a next-of-kin)

FOR CREMATION

• Permit to Cremate (original)
• Identification Papers of Next-of-kin and Applicant (e.g. NRIC, passport, etc.)
• If applicant is not a next-of-kin [Letter of Authorisation], OR
• If applicant is not a next-of-kin and deceased was a resident in Stay-in-Care* or MSF residential home (RH)
