Apply In-Person

  1. The applicant shall visit the office of the Social Service Agencies which are registered as members of the National Council of Social Service (NCSS). Contact details of NCSS office Contact link
  2. Collect the application form from the office.
  3. Now complete the form with the necessary information and make sure you have all the documents in hand that are listed under the “Required Documents” section of this page.
  4. Attach the documents mentioned in the “Required Documents” section below with the completed application form.
  5. Check the application form and then submit it to concerned person.
  6. Pay the required fees to the concerned person.
  7. The officer after validating the application form and the documents will provide you with an acknowledgement/receiving slip.
  8. Once it is processed, and the card is ready, the applicant will receive an SMS or call on the registered mobile number to collect it.
  9. The applicant will be required to carry the acknowledgement/receiving slip when going to collect it from the office.

InstructionsEdit

  • The DDR ID Card cannot be transferred to another person. It can be used only by the person to whom the card was issued.
  • The benefits can be availed only by producing a valid DDR ID card.
  • The ID card does not have any cash value.
  • The DDR ID card should not be altered or modified in any manner.
  • The DDR ID card belongs to the NCSS and must be returned when requested.
  • Misuse of the DDR ID Card to get benefits may cause termination of the Card or removal of benefits without refund.
