Apply In Person

1. To Apply Full Medical Examination For Foreign Workers, the applicant has to approach Ministry of Manpower contact link
2. The applicant can download the application form from the following link
3. Read through the instruction carefully and fill the applicant form correctly, please verify that all the information is correct (Incomplete application form will be rejected)
4. The applicant has to attach the required documents that are mentioned in the “Required Documents” section of this page.
5. After completing the application form the applicant has to post out the form to Work Pass Division of ministry of Manpower. contact details of the office address was given in “Office Location and Contacts” section of this page)
6. After submitting the application form the authority will contact the applicant within 12 working days.
7. If the application meets all the requirements, the applicant has to pay the prescribed fee as per the authority request (If applicable).
8. Upon successful completion, the work permit will be granted to the applicant within the stipulated period of time.
9. The authority officer will contact the main contractor for an appointment if necessary.
10. The outcome of the application will be mailed to the company.

Instructions

• Applicant must send their workers for a medical examination by a Singapore-registered doctor within 2 weeks of their arrival in Singapore or 7 days of completing Stay-Home Notice (SHN). Bring along the medical examination form when the worker goes for the tests.

Need for the Document

• The worker must pass the examination, and you need the completed medical form to get the Work Permit issued. Those who fail the examination will have to be sent home.

• The medical examination screens the worker for 4 types of infectious diseases (tuberculosis, HIV, syphilis and malaria) and checks if they are fit to work.
