Procedure

Apply In-Person:

1. To Apply for Offsite Healthcare Services (For Healthcare Institutions), the applicant has to approach the Ministry of health link
2. The applicant has to download and complete the application form given in the following link
3. The application form can also be obtained in-person from the respective officer.
4. The applicant has to complete all necessary details in the application form.
5. After filling the application form, make sure to attach all the documents listed in the “Required documents” section of this page.
6. The applicant has to pay the prescribed fee as per the authority's request.
7. Submit the application form to the health promotion board’s email [email protected] for endorsement. HPB will send the endorsed application to the Ministry of Health for approval.
8. The authority will review the application and contact the applicant via email or SMS.
