Procedure

Apply In Person

1. To Apply For Making Of Advance Medical Directive (AMD), the applicant has to approach the Ministry of Health link
2. An applicant has to get the form from the respective office or download the form using the following link
3. An applicant has to fill in all the necessary details in the application form.
4. Make sure the applicant has all the necessary documents that are mentioned in the “Required Documents” section of this page.
5. The applicant has to attach the photocopies of the documents along with the application form.
6. Submit the completed form with the necessary documents to the doctor and meet along with the witness.

   NOTE: The one witness should be a doctor and the other one should be above the age of 21, can be the doctor’s nurse, or any other suitable person.

7. After this, the completed form should be sent in a sealed envelope by mail to the address given in the “Office Locations & Contacts” section of this page.
8. The Registrar will send you an acknowledgment when the directive has been registered.
9. The fee for this procedure has to be paid as per the authority's request.
10. The processing time for this procedure is one month.

NOTE: Applicants can send this form by fax or other means immediately after it is completed to the address given in the “Office Locations & Contacts” section of this page. If the form is faxed, the original copy should also be forwarded to the Registry

Eligibility

• Any person who has made an advance medical directive under the Advance Medical Directive Act may in the presence of at least one witness revoke the directive in writing, orally, or in any other way in which the person can communicate.

• The fee for this procedure has to be paid as per the authority's request.

Validity

• AMD is only valid when it is registered with the Registrar of Advance Medical Directives.
