Apply by Post

  1. To Apply for a Medical Certificate Review, the applicant has to post their completed application to Healthcare Improvement Scotland, Death Certification Review Service Link
  2. The applicant has to download the application form from the following link
  3. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  4. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach photocopies of the required documents along with the application form while submitting it.
  6. Submit all the documents with the completed form to the Healthcare Improvement Scotland, Death Certification Review Service.
  7. The Mailing address are provided in “Office Locations and Contacts” section of this page.
  8. The fee for this procedure is free of cost.
  9. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  10. If the application is successful, a medical reviewer will review the medical certificate and speak to the doctor who certified it.
  11. Healthcare Improvement Scotland will write to the applicant with the outcome of the review.
  12. This entire process can be completed within 14 days or as per the timeline advised by the respective department authority.

To apply for an interested person review you must have a clear link to the person who has died, for example you should be:

  • A family member
  • A healthcare professional involved in their care
  • The funeral director who arranged the funeral, or
  • The person having charge of the place of disposal.

  • The fee for Applying Medical Certificate Review is free of cost.
