Saudi Arabia - Obtain Death Certificate

ProcedureEdit

  1. To obtain a death certificate for the first or to replace a certificate case of loss, theft or destruction, an applicant must submit a completed application to the nearest Civil Affairs Office. Note: A list of contact information for different Civil Affairs Office all over the country is provide under "Office Locations & Contacts" section.
  2. Aside from the completed application form, an applicant will also be asked to submit other documents such as the death report issued by the relevant hospital, for a complete list documents needed see "Required Documents" section.
  3. Wait for instructions from the Registrar regarding when can you comeback to collect the certificate.

You may use the link provided under "Office Locations & Contacts" to get in touch with the nearest Embassy of Saudi Arabia in the country you are currently staying. The Embassy's consular service section will be able to help process your request to obtain a copy of a death certificate.

Required DocumentsEdit

Issuance of Death Certificate for Saudis:

  • Death Report issued by the relevant hospital.
  • The deceased's Civil Affairs ID card and Family Register.
  • The specified Form No. 87 duly filled.


Issuance of Death Certificate for non-Saudis:

  • Death Report by the relevant hospital certified and signed by the hospital director if the death is normal, or a letter from relevant authorities.
  • The deceased's passport and Residence Permit (Iqama) and copies of both.
  • The Death Certificate must be handed over to the deceased's legal representative after taking the necessary measures.
  • The specified Form No. 87 duly filled.


Replace lost Death Certificate for Saudis:

  • Application to Civil Affairs.
  • Attendance of the guardian or one of the legitimate heirs with a copy of his ID card.
  • Submit a copy of the deceased's Family Register.
  • A copy of the deceased's Family Book.
