Apply for a Legal Name Change (NOT due to marriage, divorce, annulment or death of spouse
- If the name change does not involve marriage, divorce, annulment or the death of your spouse or to apply for a legal name change for your child(ren) or legal dependent(s), call the Customer Support Team contact number, 1-855-eHS-LINK (347-5465). A support representative will explain to you the process and send you the application form.
- You may also visit the Regina Customer Service of the Vital Registry to speak with a support representative and obtain the application form in-person at 2130 11th Avenue, Regina.
Register a Legal Name Change
- Obtain Consent:
- If you are already married and you want to legally change your name and the name of your spouse, you must obtain the written consent of your spouse.
- To legally change the name of a child aged 14-18, a written permission from the child is required.
- You need to obtain a written consent from someone who has custody of the child or legal dependent.
- If it is impossible for you to obtain a written consent, a court order dispensing with consent may be submitted instead.
- Prepare your application form, and all documents you are required to submit.
- You can send your application via post to the Regina Customer Service Centre or personally submit them at the 2130 11th Avenue, Regina. You must enclose your payment when you send or submit your application.
- The law states that, registered name changes are usually published in the Saskatchewan Gazette (unless the person whose name is changing is under the age of 15 or in other special circumstances).
- The first name listed on the application is charged an advertising fee on top of the basic application fee. Each subsequent name on the application is charged an advertising fee as well.
- Contact the Vital Statistics Customer Support Team or visit their Customer Service Centre, to inquire how you may obtain a copy of the Name Change Certificate.
Changing Your Surname - After You Got Married, Divorced or Widowed
- You do not need to contact the Vital Registry to inform them about your change of name when you get married, divorced or widowed, but there may be a need for you to inform the bank where you keep a personal account, the Saskatchewan Health or SGI about your name change, so they can change how your name appears in their systems.
- Informing the said agencies about your name change may require you to present certain pieces of identification or supporting documentation to support your change of name application.
- If you have a common-law spouse and you want to change your name, you must file a declaration to that effect with Vital Statistics.
Changing Your Surname After Marriage
- After marriage you may change your surname to:
- the surname of your spouse
- a hyphenated form of your surname and the surname of your spouse note that a surname can only be composed of two separate names joined by a hyphen
- your pre-marriage surname
- your birth surname
Note:
- No need to inform the Vital Registry about the name change.
- If you want to change your surname after you get married in a way that does not accord with the rules set out above, you will need to apply for a legal name change.

Documents Needed to Register a Legal Name Change:
- Your proof of residency and those whose name appears on the application copies of Saskatchewan Health Cards usually suffice.
- An official birth certificate for each person whose name is to be changed as long as those birth certificates were issued in Canada or the United States if a person was born elsewhere, official Certificates of Canadian Citizenship or official immigration documents must be submitted).
- A completed Change of Name Application form and the written consent of those whose names will be affected by the application (if required). A Change of Name Application form may be obtained by contacting the Vital Registry Customer Support Team toll-free at 1-855-eHS-LINK (347-5465) or visit their Regina Customer Service Centre at 2130 11th Avenue, Regina, Saskatchewan S4P 0J5.
- Official marriage certificate (if you are married).
- Official death certificate (if you are widowed).
- Official or a certified photocopy of the Decree Absolute or Certificate of Divorce (if you are divorced).
- An official or a certified photocopy of any agreement, judgement, Decree Nisi or other court order granted with respect to the custody of any child(ren) named in the application.
- An affidavit signed before a Notary Public, Justice of the Peace, or Commissioner for Oaths declaring that the statements made in the application for change of name are true.

EligibilityEdit
There's no need for you to contact the Vital Statistics Registry to request a name change when you:
- legally got married (if you were born in Canada)
- applied for a divorce
- applied for a legal annulment of marriage, or
- are widowed
Note: If you were not born in Canada, you will need to apply for a legal name change to change your landed residency card or citizenship card.
Individuals who may legally change their name:
- applicant must be 18 years of age or older
- current resident of Saskatchewan and have a valid Saskatchewan Health Card as proof of residency
- have been a resident of Saskatchewan for at least three consecutive months in the 12-month period before applying for the name change, and eligible to remain in Canada.
For those who are born in another Canadian province or territory and you change your name in Saskatchewan, eHealth Saskatchewan will notify the jurisdiction in which you were born of your legal name change. You will then be able to contact the vital statistics department in your birth province to request a new birth certificate from them with your new name on it.
You may also apply to change the name of your child(ren) or legal dependent(s)if:
- you have legal custody of the child or legal dependent.
- you obtained a written permission from someone who has custody of the child or legal dependent, and
- the child or legal dependent is under 18 years old
If the child or legal dependent is 13 years old or younger, you do not need to get the written consent of the child as part of the application process.
If the child or legal dependent is 14 years of age or older, you will need to obtain a written consent from the child as part of the application process.
There is a special application process to change the given name(s) of a child under the age of one (1).

Note: No advertising fee charged for children under 15 years of age.
