This procedure explains various ways to Amend or Update Your PhilHealth Member Data Record (MDR) in SOCCSKSARGEN.
Apply In PersonEdit
- Here's how you can amend or update your PhilHealth Member Data Record (MDR). First, download the PhilHealth Member Registration Form (PMRF). A link is provided under 'Documents to Use' section which you can click to download the form.
- Complete the application form. For the purpose, don't forget to check the "UPDATING/AMENDMENT" box. In case you do not have a printer at home, don't worry forms are also available at the PhilHealth office.
- After completing the form, go to any PhilHealth branch and submit your application and other supporting documents.
- Wait as the officer validates your information and encode it on the PhilHealth database.
- After updating your Member Data Record (MDR) the officer will issue a new MDR and PhilHealth ID.

- If you are changing your civil status from single to married, you may be asked by the PhilHealth officer to submit a photocopy of your marriage certificate.
- If you are correcting your name, birthdate, sex etc. on record, you may be asked to present your birth certificate or other legal documents that contains the correct information.
